Why was my direct debit payment declined?

What is a declined direct debit payment, and what should I do in this situation? A direct debit payment can be declined for various reasons. Here are the main possible situations:

Insufficient funds in the account (NSF)
Account blocked or restricted by the financial institution
Incorrect banking information (invalid account or routing number)
Account closed
Direct debit declined by the financial institution

Why am I being charged a fee?
When a direct debit is declined, a fee of $23 (taxes included) is automatically added to your balance. This is a standard fee used to cover the processing costs associated with the declined payment.
